We encourage you to take an active role in your care. Being prepared and asking questions ensures the best possible outcome and successful recovery.

Ask Questions
We understand that hospital stays, surgeries and procedures can cause anxiety. Ask us whatever questions you have, as many times as you need to. We’re happy to share our knowledge and experience about what you can expect during your stay.

Bring a List of Current Medicines
Be sure to tell your doctor about all the medicines you take. This is very important information as certain medications can interfere with others. Be prepared to share:

  • Herbal supplements
  • Over-the-counter medications (ibuprofen, allergy medicine, etc.)
  • Prescription medicines
  • Vitamins

Make Your Allergies Known
Avoid complications by making sure your doctors and nurses know about any drug, food, skin or other allergies.

All Test and Procedure Results
Ask for every test and procedure result and talk to your doctor about what they mean for your care.

Understand Your Home Care
If you're unsure about changes in medications or activity levels, find out before you leave.

To ensure your admission to the hospital goes smoothly, please bring the following:

  • A photo ID (driver's license or passport)
  • A list of all your current medicines
  • Any forms your primary care provider gave you to bring to the hospital
  • Any relevant medical insurance information, including insurance cards, pre-certification and any other documentation required by your insurer
  • A list of emergency contact numbers

Please feel free to bring personal items, such as pajamas, slippers, a bathrobe, socks, reading glasses, books, photographs or anything that would make you more comfortable.

However, we ask that you keep these to a minimum as there is limited space for personal items in patients' rooms. In addition, please note that UChicago Medicine AdventHealth Bolingbrook is not liable for personal belongings in the event of theft or damage.

When your surgeon schedules your procedure, a representative from the Pre-Admission Testing Center (PATC) will contact you to ask a series of health-related questions to individualize your care plan. We'll schedule an appointment before surgery if you need blood work or additional testing.

We'll ask you to arrive two hours before your surgery start time. After you check in at the information desk, you'll be directed to the Day Surgery Center on the hospital's first floor and escorted to your private room.

This time is used to review your admission history for any changes, obtain any additional testing ordered by your surgeon or anesthesiologist, perform pre-operative patient preparations and educate you on what to expect before and after your surgery.

Ease Your Mind Ahead of Time

A hospital stay can bring many visits and services from multiple medical providers. We know it’s hard to plan ahead for an emergency hospital visit, but if you have a scheduled healthcare service that requires a hospital stay, we recommend contacting your anticipated healthcare practitioners within the hospital to confirm they accept your insurance provider. You can also get information about billing practices and an estimate for your upcoming services ahead of time to help avoid any unexpected challenges and fees.
